Innovating healthcare delivery models that save more lives

AIDS Healthcare Foundation (AHF) is a global nonprofit organization providing high-quality HIV care, medicine, and advocacy to over two million people in 47 countries. Founded in 1987, AHF began as a network of hospices committed to “fighting for the living and caring for the dying.” Over time, under the leadership of its president and cofounder Michael Weinstein, AHF expanded, turning hospices into healthcare centers and building a new paradigm for HIV care both in the United States and around the world. It is now the largest AIDS organization in the world.

 

Focused on creating innovative ways of treating and addressing barriers to care, AHF operates a network of pharmacies, thrift stores, health and wellness centers, affordable housing locations, and food-service programs.

 

AHF’s core revenue is generated by billing insurance providers for HIV patient medications based on a complex cost-of-sales structure tied to approximately 250,000 monthly prescriptions. As AHF expanded its HIV-focused services and healthcare centers across 47 countries, the organization urgently needed to digitalize its operations. With a focus on finance, AHF wanted to simplify managing global financial data across these countries and streamline communication and business processes between bureaus while integrating line-of-business subsystems. The scope of the project involved modernizing financial management planning, reporting, and tracking workflows, as well as providing real-time data access and transparency of cost sources in high volumes of sales transactions. It also included automating and centralizing record sharing for patient advocacy services.

Driving growth and profitability with finance-led ERP offerings

To achieve its aims, AHF decided to adopt advanced cloud ERP through the GROW with SAP journey. Providing a guided pathway to SAP Business Suite, GROW with SAP enables AHF to adopt and expand its SAP ecosystem with robust, integrated solutions that drive innovation and growth.

 

AHF could now streamline and align its regional bureaus worldwide with updated financial management and associated data workflows by migrating to SAP S/4HANA Cloud Public Edition integrated with the SAP Concur portfolio. And with SAP Business Technology Platform (SAP BTP), AHF gained a consistent framework to simplify extending and customizing its solutions. So it also adopted the SAP Analytics Cloud solution on SAP BTP and integrated other software suites including SAP Ariba and SAP SuccessFactors solutions.

 

An important aspect of AHF’s project was setting up its chart of accounts, so the nonprofit sought help from SAP partner Nagarro to expertly guide the project. Nagarro took charge of setting up SAP S/4HANA Cloud Public Edition, migrating AHF’s data from the legacy ERP application and handling the integration of subsystems relative to each line of business. This work involved guiding stakeholders through fit-to-standard workshops to align processes and workflows with best practices. Nagarro was also responsible for managing a specialist integrator to handle the implementation of SAP Ariba solutions.

It was important to understand the differences between the SAP ERP application and the new environment so we could take advantage of functionality for a structured cost center and work breakdown that we hadn’t had in the past. Nagarro steered conversations with our internal teams to help them understand the differences between the two systems and what the prospect of faster decision-making through access to real-time data would mean for our organization.”

Helping more patients globally receive critical care faster

Through the GROW with SAP journey, AHF transitioned to SAP S/4HANA Cloud Public Edition. Using SAP BTP to enable SAP Analytics Cloud and integrations with SAP SuccessFactors, SAP Concur, and SAP Ariba solutions, AHF now capitalizes on real-time transaction visibility. The integration across SAP Business Suite provides a unified and intuitive user experience on multiple devices, enhancing usability and satisfaction by minimizing disruptions and promoting a cohesive experience. The new setup encompasses 23 domestic company codes, 65 global entities in group reporting, 12 integrated systems, 938 profit centers, and 166 plants.

 

The use of SAP Analytics Cloud allows for real-time insights and accurate tracking of sales and costs at scale. As such, AHF is functioning more efficiently as a global entity despite cost fluctuations and high transaction volumes globally. And thanks to planning capabilities in SAP Analytics Cloud, teams across AHF are now better equipped to make more-informed financial decisions.

 

At the core of AHF’s operations, accurate financial data is shared among AHF, strategic partners, and patients in its distributed pharmacy dispensing system. By integrating all lines of business and harmonizing financial data management processes—covering sales, costs, and accounts payable—AHF has enhanced workflows and transparency, with real-time access to critical information directly from the system.

 

With access to modern financial management data, administrators and operational staff can make quicker decisions advocating patient care using real-time data rather than wasting time looking for vital information. As health information processing varies by location, need, application, and settings, sharing patient data between systems—such as identification, medical records and history—streamlines administration. With HIV- and AIDS-related infection still growing at an alarming rate, especially in children, optimizing processes globally helps ensure more patients receive critical care faster.

 

Honig Mojica comments on what the finance transformation means to business at a reporting level: “We used to take about eight days to soft-close the accounts and then another few days to analyze the variances and make sure everything was correct. With SAP S/4HANA Cloud Public Edition, we can shrink this timeline significantly. And in terms of those 250,000 monthly prescriptions, we have the information in the system to understand revenue and cost at a per-prescription level. Running that type of reporting in our legacy environment would have taken hours, if not days. Now it takes just minutes.”

Saving more lives by enhancing operations as a global entity

With the successful implementation of SAP S/4HANA Cloud Public Edition, AHF is in a strong position to achieve its ambitious growth plans. It is also looking to maximize use of the reporting capability offered by the SAP Analytics Cloud solution and take advantage of SAP Fieldglass solutions to manage its contingent workforce. In addition, through integration of SAP S/4HANA Cloud Public Edition with SAP Concur solutions, AHF is expecting to see a significant improvement in auditing grants and fund management.

 

While the recalibration of its finance operations has yielded significant benefits across the organization, AHF is now exploring using generative AI and the Joule capability to unlock further efficiencies in financial accounting and workforce management.
