Please complete a CPE credit request form (PDF, 5 KB) in its entirety and fax it to the number provided on the form in order to receive continuing professional education (CPE) credits for a qualifying class. Please keep in mind that not all courses qualify for CPE credits. The course description will outline whether CPE credits are available and will specify the number of credits given for that particular course.

An SAP representative will verify your participation and will process your request after we receive your completed form. You should receive a certificate within two weeks of faxing form.
