SAP America provides dental coverage through a Preferred Provider Organization (PPO). The plan covers 100% of reasonable and customary charges for preventive care, including teeth cleanings, oral exams, X-rays, and fluoride treatment, with no deductible. In addition, the plan covers basic and major care services and orthodontia after an annual deductible of $50 per person or $150 per family is met. The annual plan maximum is $2,000 per covered individual.

Dental coverage requires a separate election and employee contribution than the health plans. You must be enrolled in one of the health plans to enroll in the dental plan.

• Basic restorative services – 80% for fillings, simple extractions, root canals, oral surgery, and denture repair.
• Major restorative services – 60% for crowns, partial dentures, and bridgework.
• Orthodontia – The plan provides 50% coverage for orthodontic services with no age limitations. The lifetime maximum for orthodontia is $1,500 per covered individual.

Benefit coverage levels are the same whether or not your dental provider is a participating provider in the PPO. However, the advantage of using a network provider is that reduced fees are negotiated with providers in the network, which means any cost-sharing portion of the bills you pay is also reduced.

Network providers will also file claims for you. Out-of-network providers normally require that you pay for their services in full and then submit a claim for reimbursement.
